The role of prostaglandins in the aetiology and treatment of erectile dysfunction

Abstract

Both animal and human penile tissue synthesize prostaglandins (PGs). Furthermore, intracavernous injection of certain PGs elicits erection in men with erectile dysfunction (ED). It is also well established that PGs are involved in the pathophysiology of atherosclerosis and diabetes mellitus (DM). Since atherosclerosis and DM are major risk factors for ED, it has been suggested that the disruption of PG synthesis in penile tissues and related vasculature may play a role in the pathogenesis of ED. In this review, we discuss the role of PGs in normal penile erection as well as on the pathophysiology and treatment.

      Prostaglandin E1 is used in the treatment of male erectile dysfunction as a suppository or as an intra-tissue injection (Urciuoli et al., 2004). Prostaglandins enhance the penetrability of blood vessels and act as a vasodilator (Khan et al., 1999). Prostaglandins in women also stimulate the uterus, and prostaglandin E leads to the contraction of proximal part of the fallopian tubes (a quarter of the length of the tube that is closest to the uterine body) (Karim and Hillier, 1974), which can increase experienced orgasm because of improved uterine and fallopian tube contractions (Traish et al., 2004).

      Prostanoids are a heterogeneous group of substances with different physiological effects on the body. Penile erectile tissue is able to locally synthesize and metabolize most of these substances (Jeremy, Morgan, Mikhailidis, & Dandona, 1986; Khan et al., 1999; Minhas, Cartledge, & Eardley, 2000; Moreland et al., 2001).       Some of them are involved in contraction of penile tissue including PGF2α and more importantly TXA2 which in turn result in inhibition of penile erection. Others are involved in relaxation such as PGE1 and PGE2 which in turn cause stimulation of penile erection [15]. In this regard, the use of nonsteroidal anti-inflammatory drugs (NSAIDs) in patients with erectile dysfunction has been always questionable as they inhibit the COX enzymes and in turn inhibit the synthesis of PGs. Determining the potential effect of COX inhibitors with varying selectivities on corpus cavernosal relaxation may help to reduce the incidence of NSAIDs untoward effects and help prescribing the appropriate COX inhibitor as anti-inflammatory agent for patients with erectile dysfunction.
