Is prostacyclin the major pro-inflammatory prostanoid in joint fluid?

Abstract

Prostanoid levels were measured in 10 samples of synovial fluid from 8 patients with rheumatoid disease. 6-Oxo prostaglandin F (6-oxo-PGF), the stable chemical hydrolysis product of prostacyclin, was present in higher concentration than prostaglandin E2 (PGE2), prostaglandin F (PGF) and thromboxane B2 (TXB2). There were significant correlations between the concentrations of 6-oxo-PGF and TXB2 (p < 0.001) and PGE2 and PGF (p < 0.01). Full mass spectra of 6-oxo-PGF and TXB2 were obtained from the joint fluid of one untreated patient. Prostacyclin may be involved in the genesis and maintenance of the acute inflammatory reaction in arthritic joints.

Synovial fluid from rheumatoid arthritis patients has higher PGI2 [142], and IP knockout mice exhibit reduced arthritic scores in collagen-induced and IgG-mediated autoantibody-induced arthritis [143,144], illustrating a potentially detrimental role of PGI2 in the pathogenesis of this disorder.     Thromboxane A2 and prostacyclin in particular are counterbalancing mediators, the former stimulating platelet aggregation and the latter inhibiting it.58-60 Prostacyclin is present in mouse and human arthritic joints,62,63 and we have demonstrated that platelets produce prostacyclin with synoviocytes using a transcellular mechanism during arthritis.19,62,64 The balance between such mediators may thus impact platelet functions and guide the fate of the vasculature: its leakage or its preservation.
