Examination stress, platelet peripheral benzodiazepine binding sites, and plasma hormone levels

Abstract

The maximal binding capacity (Bmax) and the equilibrium dissociation constant (KD)values for [3H]PK 11195 binding to peripheral benzodiazepine binding sites were measured in the platelets of subjects immediately after examination stress and 10 days later, as well as in unstressed controls. Increased (52%; p < 0.05) Bmax, but unaltered KD, values were observed immediately after the stress as compared to the controls. Ten days after the stress, a slight decrease (15%) in the number of peripheral benzodiazepine binding sites was observed, but their values remained higher (29%) in comparison to unstressed controls. Levels of plasma stress hormones (cortisol, growth hormone, and prolactin) did not differ in the stressed subjects from those of the controls.

      However, changes in steroid blood levels do not allow definite conclusions on central changes, which may arise at a different timing (Purdy et al., 1991). While the stress related increase of allopregnanolone is in accordance with findings from preclinical studies (Barbaccia et al., 1996; Purdy et al., 1991), prior research on TSPO levels in platelets showed increased levels after oral examinations (Fortuyn et al., 2004; Karp et al., 1989). Interestingly, those studies did either report no changes of stress hormones like cortisol or absent coherence between changes of TSPO and cortisol.

      Drugan, Basile, Crawley, Paul, and Skolnick (1986) found that inescapable shock reduces [3H]Ro5 4864 binding to TSPO in the rat. Our research on blood cells and the brain suggests that TSPO density changes due to acute and chronic stress are different in that acute stress often results in enhanced TSPO density, and chronic stress in decreased TSPO density (Karp, Weizman, Tyano, & Gavish, 1989; Weizman et al., 1994, 1987). In general it is thought that TSPO–steroid interactions form the basis for this general TSPO “stress response” function (Veenman & Gavish, 2012).

      The presence of these receptors in blood cells, such as platelets or lymphocytes, which undergo the same regulation and changes as in other tissues, such as in the brain (Drugan et al., 1986), has offered a peripheral tool for studies in humans. Stressful situations, such as that of sitting for an exam (Karp et al., 1989) or a parachutee-training course (Dar et al., 1991) influence the density of the receptors in the sense that an acute stress seems to provoke an up-regulation, while a repeated stress provokes a down-regulation (Drugan et al., 1987; Okun et al., 1988; Hu et al., 1989; Jung-Testas et al., 1989; Purdy et al., 1990). There is also a growing amount of literature regarding the changes of peripheral BDZ receptors in some psychiatric disorders, such as generalized anxiety disorder (GAD), obsessive-compulsive disorder (OCD), schizophrenia, Alzheimer's and Parkinson's diseases, bipolar disorder and post-traumatic stress disorder (PTSD) (Bidder et al., 1990; Bonuccelli et al., 1991; Rocca et al., 1991; Marazziti et al., 1994a, 2005; Gavish et al., 1996, 1999).
