Reduction of immune function in life stress and depression

Abstract

Reduced cell-mediated immune function has been found in depressed patients and in distressed persons undergoing threatening life events. The present study examines the interaction between severe life stress and major depression to produce immune alterations in 36 matched pairs of hospitalized depressed patients and nondepressed controls. Both major depressive disorder and the presence of threatening life events in control subjects are independently associated with a 50% reduction of natural killer (NK) cytotoxicity. A decrease in natural cytotoxicity is significantly associated with depressive symptoms but not with age, alcohol consumption, or tobacco smoking. These findings of altered immunity provide further evidence that the physiological responses in chronic stress parallel those found in the syndrome of depression.

      The hypothalamic-pituitary-adrenal-gonadal axis, which exerts important control on the adaptive immune system, differs between males and females. Exercise, stress, and depression all downregulate immune function (Irwin et al., 1990). As a rule, estrogenic hormones upregulate and androgenic hormones downregulate the cellular effectors of human adaptive immunity: lymphocytes, macrophages, and dendritic cells (Fish, 2008; Oertelt-Prigione, 2012a).

      A chronic stress-induced decrease in baseline leukocyte numbers and leukocyte mobilization and trafficking from the blood to other body compartments is an important mediator of stress induced suppression of immune function (Dhabhar and McEwen, 1997). In human and animal studies, chronic stress has also been shown to suppress different immune parameters examples of which include: CMI (Kelley et al., 1982), antibody production (Edwards and Dean, 1977; Fleshner et al., 1989), NK activity (Irwin et al., 1990; Bartrop et al., 1977; Kiecolt-Glaser et al., 1984; Cheng et al., 1990), leukocyte proliferation (Bartrop et al., 1977; Cheng et al., 1990; Regnier and Kelley, 1981), skin homograft rejection (Wistar and Hildemann, 1960), virus-specific T cell and NK cell activity (Bonneau et al., 1991), and anti-mycobacterial activity of macrophages from susceptible mouse strains (Brown and Zwilling, 1994). Accelerated biological aging is another important mechanism through which chronic stress suppresses/dysregulates immune function.
