Expression of thyrotropin-receptor mRNA in healthy and Graves' disease retro-orbital tissue

Abstract

It is not clear wether the ophtalmopathy present in Graves' disease is related to autoimmune reactions to common thyroid/orbit components or is due to specific ocular antigens unrelated to the thyroid. In Graves' disease, serum antibodies to the thyrotropin receptor (TSH-R) can stimulate thyroid function, but no link between the TSH-R and ocular tissues has been found. We have shown, by polymerase chain reaction amplification of specific cDNA, that mature TSH-R mRNA is expressed in the retro-orbital tissue of both healthy subjects and patients with Graves' disease. Of other tissues and cells tested (fibroblasts, lymphocytes, muscle, and thyroid), only thyroid tissue expressed the TSH-R mRNA. Lancet 1993; 342: 337–38

    Using axial magnetic resonance imaging analysis, it has been determined that for patients with TAO, the orbital fatty tissue volume is more closely related with the degree of exophthalmos than extraocular muscle [6]. Thyroid-stimulating hormone receptor (TSHR), the primary autoantigen in TAO, was first validated by identifying TSHR expression in retro-orbital tissue of cultured orbital fibroblasts from patients with TAO using polymerase chain reaction (PCR) and liquid hybridization [7,8]. Notably, adipogenesis of orbital tissues is positively correlated with the enhanced expression of TSHR in orbital adipose/connective tissues [9].

    A shifting focus to the potential involvement of TSHR as an autoantigen shared by the thyroid and orbit was provoked by the successful molecular cloning of the receptor [5]. Feliciello and colleagues reported soon thereafter detecting TSHR mRNA in orbital tissues from both healthy donors and those with TAO [51]. Its expression in orbital fibroblasts was subsequently detected [52], suggesting that these cells might be responsive to TSH.
